Atorvastatin and Macrolides:

The Co-Administration of both Atorvastatin and (Clarithromycin or Telithromycin) should generally be avoided (Category D) because they increase the serum concentration of Atorvastatin.

 

What if one of them has to be used ?

Clarithromycin: limit Atorvastatin dose to 20 mg and monitor for S&S of statin toxicity (muscle pain, renal dysfunction)

 

Telithromycin: D/C Atorvastatin for the duration of Telithromycin therapy.
